The Virgin and Child and Saint Joseph are flanked by Saints Stephen, John the Baptist (left), Jerome and Nicholas of Bari (right). God the Father appears above between two angels. The figure behind Saint Stephen is probably a shepherd. Each pilaster is painted with three saints - on either side at the top are the Angel Gabriel and the Virgin Mary (forming the Annunciation); in the centre are Saint Peter and Saint Paul; and below are Saint Francis and Saint Lucy.
The predella depicts the Agony in the Garden with Saints Peter, James and John; the Betrayal of Christ, with Saint Peter cutting off the ear of Malchus; the Crucifixion with the Virgin Mary, Saint John the Evangelist and Mary Magdalene; the Deposition with the Virgin Mary, Saint John the Evangelist, Mary Magdalene, and probably Joseph of Arimathea; and the Resurrection. At either end of the predella are the coats of arms of the Cerretani family and of a relation of the Piccolomini of Siena. The intended location of the altarpiece has not been identified, perhaps in part because it was previously attributed to Pacchiarotto (1474-1540 or later), but Orioli was active at the end of the fifteenth century.
